For millions of people living with chronic migraine headaches, October 2010 marked a turning point. That is when the FDA granted approval to therapeutic Botox (onabotulinumtoxinA) as the first preventive treatment ever designed specifically for chronic migraine. Health Canada officially approved Botox (onabotulinumtoxinA) for the prevention of chronic migraines on November 14, 2011. And it happened because of one landmark research program: the PREEMPT trials.

Behind that approval was one of the most rigorous research programs in headache medicine history: the PREEMPT trials.

Understanding what the PREEMPT trials were, how they were conducted, and what they proved is essential for any chronic migraine sufferer considering therapeutic Botox because the strength of that research is exactly why neurologists and headache specialists around the world trust this treatment today.

WHAT WERE THE PREEMPT TRIALS?

PREEMPT stands for Phase III Research Evaluating Migraine Prophylaxis Therapy. The program comprised two parallel double-blind, placebo-controlled clinical trials: PREEMPT 1 and PREEMPT 2, representing the largest and most comprehensive chronic migraine study ever conducted at the time of publication. Participants were adults suffering from chronic migraine headache disorder, defined as 15 or more headache days per month with each episode lasting four hours or longer.

America & Europe

Every participant received either onabotulinumtoxinA or a placebo injection every 12 weeks across a rigorous 24-week double-blind phase. Neither patients nor treating physicians knew which group each participant had been assigned to eliminating bias and ensuring the integrity of every result.

In PREEMPT 2, patients in the Botox group experienced an average of 9.0 fewer migraine headache days per month, compared to 6.7 for the placebo group. For a chronic migraine sufferer losing half their month to debilitating head pain, nausea, and light sensitivity, that difference was life changing.

Importantly, the PREEMPT trials also established the precise injection protocol that headache specialists use in every clinic today: 31 injections across seven muscle groups of the head and neck using an ultra-fine 30-gauge needle, repeated every 12 weeks. This standardized approach was not arbitrary. It was the result of carefully designed research across more than a thousand patients, refined to deliver the most consistent, evidence-based migraine headache relief possible.

The PREEMPT Legacy Keeps Growing

The evidence did not stop at Health Canada and FDA approval. Long-term follow-up studies including COMPEL and FORWARD have since confirmed that the migraine prevention benefits seen in the PREEMPT trials not only hold up over multiple years of treatment, but they also improve. Patients who remained on therapeutic Botox across successive 12-week cycles continued to see progressive reductions in migraine headache frequency and severity, reinforcing that this is not a short-term fix but a sustainable, cumulative migraine management strategy.

The PREEMPT program also confirmed a critically important safety finding: none of the 1,384 study participants developed neutralizing antibodies to Botox at study completion, and the FDA noted no confirmed serious cases of toxin spread at the recommended migraine dose. For chronic migraine sufferers concerned about long-term medication safety, this evidence base provides genuine reassurance that oral preventive medications simply cannot match.

The PREEMPT trials are the reason therapeutic Botox is not guesswork, it is goldstandard, FDA-approved, evidence-based chronic migraine prevention, validated across more than a thousand patients at 122 sites worldwide. If you experience 15 or more migraine headache days per month, ask your neurologist or headache specialist whether you qualify. The research has already been done. The answer is waiting for you.
